To clarify the morphological changes of human diabetic gastric mucosa in vivo and in vitro and to investigate the mechanism of the development of gastric mucosa lesions in diabetes. The proximal portion of human stomach from 8 normal subjects and 26 diabetic patients were removed for examination. Both were divided into 4 parts: mucosa, submucosa, muscularis, serosa. The specimens were incubated with high concentration of D-glucose for 0, 15, 30, 60 minutes and 1, 2, 3, 4, 6, 8, 12, 24 and 48 hours. After incubation, the specimens were fixed and stained with HE and PAS-alcian blue. The sections were evaluated for cell density, villous height and width, and vascular congestion, mucin secretion and PAS-alcian blue positive cells. After incubation with glucose, the glandular cells in gastric mucosa of normal subjects were arranged in 3 dimensional meshwork structure. Their villous height and width increased. The ratio of villous height/width decreased gradually. In the specimens from diabetic patients, there were no significant changes in both villous height and width. In the course of the incubation, the density of epithelial cells decreased gradually in both normal and diabetic groups. The ratio of villous height/width increased gradually in normal subjects. In the specimens from diabetic patients, the villous height and width decreased in the early stages of the incubation. However, in the late stages of the incubation, the ratio of villous height/width increased gradually. In the diabetic group, the ratio of villous height/width increased gradually after the glucose incubation for 8 hours. The ratio of villous height/width decreased gradually in the normal group after the glucose incubation for 1 hour. The ratio of villous height/width decreased gradually in the diabetic group after the glucose incubation for 4 hours. The decrease of villous height and width in gastric mucosa is an early morphological change in diabetes.
